Health Risk Assessment (HRA) Document Type H Changes in MMIS

Discussions and planning between DHS managed care staff and care coordination representatives for the past several years were finalized on June 28, 2018 with the implementation of MMIS changes for Health Risk Assessments (HRAs) entered as document type H. The changes were not intended to be implemented prior to August 1, 2018, but other MMIS work resulted in moving these changes into production early. New fields appear in MMIS when selecting Document-H for data entry, but these new fields are not to be completed until the Activity Type Date is August 1, 2018 or greater.

MMIS changes implemented on June 28, 2018:

Some fields that were mandatory in the past are no longer mandatory when completing a LTC SDOC (such as diagnosis codes). The cursor moves past the fields that are not required. 

Only these fields are required when the Assessment Result is 35 for Health Risk Assessments:

  • Date of birth
  • Activity type
  • Activity type date
  • County fields
  • LTCC county
  • Care coordinator NPI number
  • Team
  • Current housing (completed for SNBC only)
  • ADL dressing through behavior (Note the behavior field is only completed for SNBC)
  • Hearing, communication, vision
  • Insulin (complete only for SNBC)
  • Assessment result
  • Effective date
  • Program type 18 for MSHO/MSC+ or 28 for SNBC 

The ALT5 screen is skipped

Beginning with Activity Type Date August 1, 2018, new fields will be mandatory for HRAs:

  • Dental concerns (screen ALT2) MSC+, MSHO, and SNBC
  • Do you have a dentist (screen ALT2)  MSC+, MSHO, and SNBC
  • Family planning (screen ALT3) SNBC ONLY      
  • Sex Activity (screen ALT3) SNBC ONLY
  • Return to community (screen ALT4) SNBC ONLY

Also, new values for the ADL and IADL fields will be used, which are required to be completed August 1, 2018.

Other activity intended to support the original August 1, 2018 implementation date is near completion:

  • New assessment and screening document forms 3427H and 3428H to replace the current use of DHS-3428, 3428A, and 3427 for HRA screenings will be in eDocs soon
  • MMIS manuals DHS-4669 and DHS-5020A will be in eDocs soon
  • Web-based MMIS training that will include a new session 8-B that reviews the HRA fields is near completion
