Description

Minnesota’s Special Needs Basic Care health plans are working collaboratively with Minnesota Department of Human Services (DHS), including the Special Needs Purchasing team and the Direct Care and Treatment clinics (DCT), on an improvement project designed to increase the number of SNBC members accessing their dental benefit and reduce barriers to dental care for people with disabilities. The project includes several intervention strategies to support SNBC members, including Dental Case Management.

 

This training will focus on the Case Management intervention, how this will impact the work of SNBC Care Coordinator/Case Managers, and what resources the Managed Care Organizations are working on to help support Care Coordinator’s work with members.

 

Target Audience     SNBC Care Coordinators/Care Navigators

 

Learning Objectives

Goal 1:   Participants will understand background and strategies being planned as part of the SNBC Dental Access and Improvement Project.

Goal 2:   Learn about the Case Management initiative planned to help connect SNBC members to dental care.

Goal 3:   Understand the implementation timeline for the SNBC Dental Access Project and the Case Management intervention.

Goal 4:   Understand how this will impact the work being done by SNBC Care Coordinators/Case Managers, and how you may be able to partner with the project.

Goal 5:   Understand resources that will be made available to Care Coordinators/Case Managers working with SNBC members.

 

Pre-registration is required.                                    

Each person needs to  register.

Registration closes at 5:00 p.m. on the Monday prior to the session. By registering, you understand that the session will be videotaped with the possibility of your visual and audio likeness being recorded for reproduction and distribution.

 

Handouts and Resources

Presentation handouts and resource materials will be distributed to registered participants via email attachment 1-2 days before the scheduled session. Print and bring your own copies to the session. Copies may not be available at the site. Those individuals interested in receiving handouts only, can make this request via the registration survey.

 

Cancellation

Videoconferencing sites without any registered participants prior to the registration close date will be cancelled. If you register, but are unable to attend, please cancel your reservation through the web link using your confirmation number prior to the registration close date. This allows others to participate in the training.

 

Certification

Certificates for continuing education are available for participants at the site. Note:

Videoconference training sessions have not been submitted for prior approval to any licensing board.
