Using person-Centered Practices in Support Planning

Would you like to improve your person-centered practices? Are you ready to show the Lead Agency Review team that you are following the Person-Centered, Informed Choice, Transition protocol? Join DHS representatives for a one-day session where you’ll learn new skills and receive support to:

  • Understand the criteria used by the lead agency review team to evaluate a person centered support plan.
  • Integrate person-centered practices into assessor and case manager responsibilities and workflows
  • Develop person-centered support plans, such as the community support plan (CSP) and the coordinated services and supports plans (CSSP)
  • Support people so they can lead lives that are meaningful to them.

  • June 28, Minneapolis

Time: 9 a.m. - 4 p.m.

Audience: Tribal and county lead agency and contracted case managers, MnCHOICES mentors, certified assessors, supervisors and others with an interest in person-centered practices
