The final Minnesota Accountable Health Model task force meeting will be a joint meeting of both the Community Advisory and Multi-Payer Alignment task forces on Wednesday, April 19 from 9 a.m. to noon at the Amherst H. Wilder Center, 451 Lexington Parkway N., St. Paul, MN 55104.  All previous task force members and sub-group members are invited to attend and be recognized for their contributions to the Minnesota Accountable Health Model. 


Vendor selected to develop eLearning modules for BHH, IHP, HCH

The Minnesota Department of Human Services (DHS) has selected LearningLens, Inc. to develop, produce and deliver up to 45 accessible eLearning training modules to support Minnesota's Integrated Health Partnerships (IHP), Behavioral Health Home Services (BHH), and Health Care Homes (HCH).  LearningLens will work with DHS to develop online learning tools to provide support and information about IHP, BHH and HCH program policies, procedures and practices.

This project is part of a $45 million State Innovation Model (SIM) cooperative agreement, awarded to the Minnesota Departments of Health and Human Services in 2013 by The Center for Medicare and Medicaid Innovation (CMMI) to help implement the Minnesota Accountable Health Model.


Gray Plant Mooty Launches Foundations in Privacy Toolkit

Minnesota Law Firm, Gray Plant Mooty (GPM) has developed a "Foundations in Privacy Toolkit" to help address many of the common legal and operational challenges of exchanging patient health information amongst different providers in Minnesota. The Minnesota Department of Health funded GPM's project to analyze legal barriers and develop the toolkit. The toolkit and other health information privacy resources are available at http://www.health.state.mn.us/e-health/privacy/index.html.  

The toolkit contains the following types of material, organized by subject area:

  • Template policies and procedures
  • Template agreements
  • Flow charts
  • Checklists

The policy and procedure documents are available in multiple formats that can be customized and implemented as part of an organization's privacy compliance efforts. The flow charts and checklists can be used to analyze business relationships and unique disclosure situations, the template agreements can be used to guide negotiations and simplify execution, and all of the documents can be used in workforce training.

Gray Plant Mooty is also preparing in-depth webinar trainings on certain topics related to the Toolkit. The next webinar provides strategies for responding to a HIPAA breach is available on-demand at the Gray Plant Mooty website.
