The
MN Department of Human Services (DHS) has issued a request for proposals for Additional PHR Community Collaboratives.
The Collaboratives will demonstrate an electronic Personal Health
Record (PHR) for Minnesota beneficiaries of Long Term Services and Supports
(LTSS) funded by Medical Assistance (MA), and will participate in pilot execution
and evaluation of a nationally developed electronic LTSS (e-LTSS) standard.

The demonstration is funded by a federal Testing Experience and Functional Tools (TEFT) grant from the Centers for Medicare & Medicaid Services (CMS).
