My Right to Appeal

The brochure, My Right to Appeal DHS-1941 (PDF), is an essential piece of information for applicants and recipients because it informs them of their right to appeal.  DHS recently updated the brochure to explain there is an option to have a hearing before the judge in-person or by phone

DHS-2727 Repurposed

The DHS-2727 (PDF) is now the Long-Term Services and Supports Assessment and Program Information and Signature Sheet.  Previously, it served as the Application for Title XIX Home and Community-Based Waiver Services and Alternative Care (135 Day Eligibility) Program Information and Signature Sheet. For guidance on all the forms used during the long-term services and supports assessment, eligibility and support planning process, check out the guidance document Forms for Long-Term Service and Supports Assessment, Eligibility and Support Planning (PDF) on CountyLink.

 New Form in the 6791 Series

There are now four separate forms in the 6791 series:

  • DHS-6791A (PDF) Community Support Plan Worksheet (update coming soon)
  • DHS-6791B (PDF) MnCHOICES Community Support Plan with Coordinated Services and Supports Form (updated May 2016 and another coming soon!)
  • DHS-6791C (PDF) Instructions for MnCHOICES Community Support Plan with Coordinated Services and Supports Form (updated May 2016 and will be updated along with revised 6791B)
  • DHS-6791D (PDF) Coordinated Services and Supports Plan Signature Sheet (new May 2016)

MnCHOICES Reassessment Implementation Schedule

DHS is in the process of reviewing data and recommendations from lead agencies, about their ability to fully implement Minn. Stat. 256B.0911, which requires the county of residence (COR) to conduct Reassessments using MnCHOICES. 

This is an update regarding timelines for reassessments being conducted in MnCHOICES.  While we consider the data and recommendations please note:

  • The need to be at 100% as of September 1, 2016 will be moved out
  • New implementation dates and expectations will be determined and communicated
  • Agencies must continue to work toward 100% use of MnCHOICES

Moving out the date for CORs does not change DHS's expectation that all agencies will continue to progress toward full use of MnCHOICES for Reassessments.  Watch for communications from the SSIS/MnCHOICES Help Desk regarding confirmation of the changes including timelines and instructions.

You know the feeling:  You're working in the MnCHOICES Assessment application and something "unusual" happens.  You already have 10,000 steps logged today so you don't want to go searching for your mentor.  Try the actions below and between each action, check to see if the issue is fixed:

Keeping Up-to-Date with eDocs

Keeping updated on form revisions can be easy! DHS provides an eDocs notification service. Just sign up to receive an email update when an eDoc is revised, added or made obsolete.  The webpage where you subscribe also has an archive of notices by month and year.

FYI

  • Within the next month MnCHOICES Administrative Contacts in counties and tribes will get a short survey.  The information gathered will help DHS begin preparations to launch the MnCHOICES Support Plan (MnSP).
  • The MnCHOICES Community Support Plan Worksheets DHS-6791A are out of stock.  If your agency needs forms before the revised DHS-6791A is available, contact your neighboring agencies to see if they have any they can share with you until the new form is available....which should be soon!

What Would You Do?

To help Qualified Candidates and Certified Assessors know if they must or should complete the TrainLink course in MnCHOICES Step 3 - Part 3:  "What Would You Do?" MNCH8014, please note:

  • All Qualified Candidates who seek certification must take the course "What Would You Do?" to complete MnCAT Step 3 requirements for certifications.  This was effective May 13, 2016 when the email message was sent.
  • All Certified Assessors who were certified during the launch of MnCHOICES and who attended an in-person learning lab presented by DHS may take the course "What Would You Do?"  It can be used as a continued learning unit (CLU) for recertification.
  • All Certified Assessors who were certified using the interim Step 3 process, meaning they did not attend an in-person learning lab presented by DHS, must take the course "What Would You Do?" as described in the Step 3 interim process.  Because they are already certified, it may also be used as a CLU for recertification.

DHS-5150 is Obsolete

DHS removed the Long-Term Care Consultation (LTCC) Cost Report for Face-to-Face Assessments of Person Under 65 Years form, (also called DHS-5150) from eDocs. Because the administrative activities of intake, assessment and support planning are reimbursed using time study methodology since October 2013, we no longer need the form.

Security Access Mentor Minder

If your agency does not have a process to monitor and update security access (PDF) for MnCHOICES, develop one now.  If your agency has a policy in place make sure it is working well.  Mentors should:

  • Always know who in your agency has access to MnCHOICES
  • Know who no longer works in your agency
  • Know who no longer needs MnCHOICES to do their work
  • Review the security roles of each person who has access
  • Work closely with your SSIS Administrator to update MnCHOICES security access and roles on a regular basis (if your agency uses SSIS)
  • Submit a User Request form to DHS with requested updates (if your agency is a non-SSIS agency)
  • Schedule time on your calendar to monitor and update MnCHOICES access and roles regularly
