Grants available to provide innovative home and community-based services (HCBS)

DSD seeks proposals from qualified responders to provide innovative home and community-based services for people with disabilities in Minnesota. These services should improve outcomes in:

  • Community integration
  • Competitive employment
  • Integrated housing.

You can view the request for proposals (RFP) on the DHS Grants and RFPs page.

This request is part of a larger effort to use innovative techniques to improve outcomes for people with disabilities. We will announce opportunities for additional innovation grants later this year.  

Responders' conference

DSD will host a virtual responders’ conference for this RFP Monday, June 6, 2016, at 2 p.m. Central Time. The responders’ conference is an opportunity for potential responders to ask questions. See the RFP for more information.
