PHR User Recruitment Is Under Way!

The Otter Tail PHR Community Collaborative has begun recruiting local beneficiaries in Otter Tail County and their legal representatives to use the Personal Health Record which is currently under development. Our goal is to have 20 beneficiaries using the PHR beginning in October, 2016.

On to Wave Four

We've divided the four year PHR for LTSS Demo Project timeline into Seven Waves. We've just completed Wave Three and are on to Wave Four (4/1 - 9/30/2016). Here's what's happening: 

  Personal Health Record for LTSS Waiver beneficiaries:

  • On April 25, 2016, DHS staff and contractors again met with Collaborative members in Fergus Falls for the fifth monthly meeting of the group. The Collaborative is developing a user engagement and training strategy for users of the PHR.
  • MN.IT@DHS staff have successfully sent data to two Integrated Health Partnerships (IHPs) with more on the way. They're ahead of schedule and progressing nicely in their IHP work. They have successfully tested transmission of some data to the RelayHealth system, and continue to work to gather data from DHS systems. We continue to plan for Release One of the PHR by September 30, 2016.

   e-LTSS Standard:

  • The Collaborative has made great progress in identifying Long Term Services and Supports (LTSS) data that each member organization collects from beneficiaries in paper or electronic form. The group has contributed enthusiastically to an analysis of 475 candidate data elements, leading ultimately to identifying 125 elements which members have indicated a desire to share among themselves. The next step is to find a way to harvest and securely share the data electronically among Collaborative members.
  • Representatives of the Collaborative and DHS staff and consultants will present findings of their work to date at the 2016 MN e-Health Summit in June.

  Beneficiary Experience Survey:

  • Minnesota completed Round One testing of the "Experience of Care" survey in 2014-2015. We will participate in Round Two testing during the final year of the TEFT Grant (beginning in April of 2017).

  Functional Assessment Standard Items (FASI):

  • Round One of FASI testing will begin in July or August of 2016. We will be working with a limited number of persons receiving MA services through the Developmental Disabilities (DD) Waiver and Brain Injury (BI) Waiver, as well as beneficiaries with Severe Mental Illness (SMI).
  • FASI is NOT a replacement for MnCHOICES – we will do only 2 rounds with limited samples to fulfill our TEFT grant obligations. What we learn from FASI may be used to inform the MnCHOICES process.

What are the PHR for LTSS Demo's goals?

  1. Demonstrate use of an untethered Personal Health Record (PHR) system with beneficiaries of LTSS;
  2. Identify, evaluate and test an electronic Long Term Services and Supports (e-LTSS) standard with the Office of the National Coordinator's (ONC) Standards and Interoperability (S&I) Framework process;
  3. Field test a Beneficiary Experience Survey within multiple CB-LTSS programs for validity and reliability; and
  4. Field test a set of Functional Assessment Standardized Items (FASI) measures for use with beneficiaries of CB-LTSS programs.
