Status of Minnesota HCBS statewide transition plan

DHS updated the Transition plan for home and community-based settings page with more information on the status of the statewide transition plan.

The statewide transition plan is a way that a state:

• Determines its compliance with the regulation requirements for home and community-based settings (42 CFR 441.301(c)(4)(5) and 441.710(a)(1)(2))
• Describes to CMS how it will comply with the new requirements.

 Minnesota submitted our proposed statewide transition plan to CMS on Jan. 8, 2015.
