2015 Regional Meetings

The Minnesota Accountable Health Model is supporting a series of events in communities across Minnesota. 

The State Innovation Model (SIM) invites grantees, residents, families, health and human service professionals and other community leaders to attend these upcoming events.  Attendees will have the opportunity to share stories, shape community priorities, and build partnerships.  Events are free and open to the public, no registration is required

Fergus Falls

Practice Facilitation Services launch throughout Minnesota

In June 2015, the Institute for Clinical Systems Improvement (ICSI), in partnership with Stratis Health and the National Council for Behavioral Health began their work to provide practice facilitation services in Minnesota. 

As part of the Minnesota Accountable Health Model, State Innovation Model (SIM) initiative, ICSI/Stratis Health and the National Council are serving 25 organizations in both urban and rural settings throughout Minnesota.  Both agencies will be providing assessments; identifying needs of organizations; working with organizations to address their priority needs and build upon their strengths; initiating in person meetings with their organizations, coaching both in person and on-line, providing bi-monthly webinars, and potentially conducting affinity groups. 

ICSI/Stratis Health is providing practice facilitation services to seven primary care/specialty provider organizations and one social service agency which have the following priority needs:

  • Chronic care management
  • Health IT
  • Health Care Home certification
  • Integration of behavioral health or alternative models of care
  • Quality Improvement
  • Total Cost of Care (TCOC)

ICSI/Stratis Health will be conducting an initial in person meeting with the following organizations on Tuesday, November 17th in Hutchinson, Minnesota.

  2. Altru Health System,  Warroad and Roseau
  3. Appleton Area Health Services, Appleton
  4. Bluestone Physician Services, Stillwater
  5. Catholic Charities of St. Paul and Minneapolis, Minneapolis
  6. Hennepin County Medical Center, Minneapolis
  7. Madison Healthcare Services, Madison
  8. Sartell Pediatrics, Sartell

The National Council for Behavioral Health is providing services to eleven community mental health centers, three federally qualified community health centers, and two social service organizations.  Priority needs that have been identified by the organizations they are working with include: 

  • Accelerating behavioral health and primary care integration
  • Data use related to analytics and interoperability
  • Identifying and assessing service costs
  • Whole health programs 

The National Council for Behavioral Health will be conducting an in person initial meeting with the following organizations on Tuesday, November 10th in Chaska, Minnesota.

This project is part of a $45 million State Innovation Model (SIM) cooperative agreement, awarded to the Minnesota Departments of Health and Human Services in 2013 by The Center for Medicare and Medicaid Innovation (CMMI) to help implement the Minnesota Accountable Health Model.

Tell us what helps your community be healthy. What is your role in contributing to community health? We know health is important to you and your community and invite you and your partners to share your stories.

The following tools are available to help you talk about your health initiatives and the impact you are making in your community:

Stories that represent a variety of successes and challenges in creating healthier communities may be chosen for further development. Selected storytellers will be contacted about opportunities to develop and record their stories to share throughout Minnesota. Additional resources may also be available to help communities engage in deeper conversations about their health.

Medtronic collaborating with the Minnesota Department of Health to address chronic disease

On October 19th, Medtronic Philanthropy announced a $2 million investment in HealthRise, a major worldwide initiative to address chronic disease that will be implemented in three areas of Minnesota – Hennepin, Ramsey, and Rice counties. 

As a prominent component of HealthRise, Community Health Workers and Community Paramedics will support community organizations as they help people living with chronic diseases such as heart disease and diabetes manage their health.  The Minnesota Department of Health (MDH) has been collaborating with Medtronic during the development and implementation of the project. MDH is glad to partner with Medtronic Philanthropy to leverage common resources and knowledge from the SIM-funded Emerging Professions work, as well as CDC-funded initiatives to address heart disease, stroke, and diabetes.  For more information on the HealthRise initiative, view the full press release at Medtronic.com.


Minnesota awards Community Engagement Initiative contract to broaden participation in health partnerships for health equity

The Minnesota Department of Health (MDH) awarded Community Blueprint a $65,555 contract for the Minnesota Accountable Health Model State Innovation Model (SIM) Community Engagement Initiative.

A key emphasis of this initiative is to strengthen community relationships and partnerships to advance health equity. Community Blueprint will facilitate a co-creation process with community stakeholders to develop a communications and sustainability plan for leveraging stories to encourage health partnerships.  

The Community Engagement Initiative goals are:

  • To support, expand and sustain interest in health integration and innovation efforts across Minnesota.
  • To build the narrative capacity of partners, providers, and communities to raise awareness of health equity and health efforts in their community.
  • To support existing and new partnerships for health in a coordinated effort to launch success stories and generate discussion about health equity.

This award was made possible through Minnesota's $45 million State Innovation Model (SIM) testing grant. MDH and the Department of Human Services (DHS) jointly received this federal grant in 2013 and are using the funds to help implement the Minnesota Accountable Health Model. The model helps providers and communities work together to reduce costs and create healthier futures for Minnesotans. It supports community and provider partnerships, team-based models of care, care coordination, Accountable Care Organizations, payment reform, and health information technology investments.

Community Blueprint is a cause marketing agency with years of experience telling authentic and engaging stories through multimedia formats such as video, web, and print.  They've traveled the state and country working with people to create campaigns and spread messages that educate, engage, and inspire.  Over the years, they have worked with government agencies, schools, and public service organizations to promote a variety of public health and social change initiatives.

Call for Presentations for 2016 HCH/SIM Learning Days

Presenters are sought for this annual statewide conference for health care providers and community organizations engaged in health reform transformation. Approximately 600 participants are expected.

Proposals are requested for beginning, intermediate and advanced sessions for learning tracks in the following areas:

  • Community Partnerships
  • Population Health
  • Quality Improvement
  • Team Based Care

Peer based, skills based, interactive session content is strongly encouraged. Submission deadline is November 25, 2015


Transforming Clinical Practice Initiative Awards

HHS announces $685 million to support clinicians delivering high quality, patient-centered care

Health and Human Services Secretary Sylvia M. Burwell recently announced $685 million in awards to 39 national and regional health care networks and supporting organizations to help equip more than 140,000 clinicians with the tools and support needed to improve quality of care, increase patients' access to information, and reduce costs. The Transforming Clinical Practice Initiative is one of the largest federal investments designed to support doctors and other clinicians in all 50 states through collaborative and peer-based learning networks.
