Pause to requests for June 2026
The Department of Children, Youth, and Families’ state-contracted qualified individual (QI) program will not be accepting new requests for Qualified Residential Treatment Program (QRTP) assessments from Monday, June 15, 2026, to Tuesday, June 30, 2026. The pause is due to the end of the state fiscal year on June 30, 2026. This pause should not impact the timely completion of QRTP assessments, as state QIs complete assessments within 15 days of request.
If agencies need a qualified individual (QI) during this time frame, agencies are encouraged to reach out to other county or Tribal agency qualified individual (QI) programs to determine if they can accept a request. If agencies submit a request for a state-contracted qualified individual (QI) from Monday, June 15, 2026, to Tuesday, June 30, 2026, the request will be declined, and the agency will be asked to resubmit their qualified individual (QI) request beginning Wednesday, July 1, 2026, if an assessment is still needed.

Agencies must complete and submit the QI Request Form (DHS-8415) to request a state-contracted QI. For more details on the referral and request process, please refer to the Referral process for the state-contracted Qualified Individual program.
New practice guide published
The Department of Children, Youth, and Families has published a practice guide on Extended Qualified Residential Treatment Program (QRTP) Placement Reviews (DHS-8120). An extended placement review is required when a child or youth’s QRTP placement extends past certain time frames. This practice guide provides information on the purpose of these reviews, including key factors, definitions and directions for preparing for, completing and approving the review.
