The Maltreatment of Minors Mandated Reporting form has been updated to reflect the transition to the Department of Children, Youth, and Families (DCYF). Please ensure you are using the July 2025 updated form and your policies reflect the new contact information.
Reminder: on June 18, 2025, jurisdiction over licensing concerns and alleged maltreatment for licensed child care centers and certified child care centers transferred to DCYF. If you wish to make a report of alleged maltreatment or licensing concerns regarding licensed or certified child care centers, please contact the DCYF Intake Unit at 651-539-8222.
