Provisional Supervision Forms are available in Online Services

As of March 27, 2025, Provisional License Supervision documents can now be accessed in Online Services.
Online Services has been updated for Provisional Licensees and Supervisors to have the ease of filling out the following forms in their Online Services Portal:
Provisional License Supervision Plan
Provisional License Six Month Evaluation
Provisional License Final Evaluation
Visit the Minnesota Board of Social Work website or Log into your Online Services Account to access these forms in Online Services.
