RENEW ONLINE:  Licensee/Applicant Log In

RENEW BY MAIL:  Renewal Application Form or contact the Board to request form by mail; submit completed form by mail with your check or money order

• RENEWAL DEADLINE: Renewal application and fee must be received in the Board Office
  by [MM/DD/YYYY] to avoid a late fee—NOT by the postmark date.
• LATE RENEWALS: A Late fee of 1/4 of the renewal fee for your [License Status] [License Type] License will be assessed if renewal application is received within 60 days after [MM/DD/YYYY].
• AUTOMATIC EXPIRATION: Your License will automatically expire 60 days after [MM/DD/YYYY], if your renewal application and applicable fees are not received
  in the Board office within that time frame.

ALTERNATE LICENSE STATUS FEES: EMERITUS ACTIVE fee is 1/2 license renewal fee for license type TEMPORARY LEAVE fee is same as license renewal fee for license type EMERITUS INACTIVE fee - $65
