Updated Affidavit of Identity Form

The Forms Committee, made up of county and local clerks, finalized minor modifications to the Affidavit of Identity.  These changes have been sent to the vendors to update their stock and the online fillable version has been updated on the Bureau of Elections' website.

These changes do not make old forms obsolete.  Any Affidavit of Identity form with a date between now and 2008 should still be accepted.  No changes were made to the Precinct Delegate Affidavit of Identity at this time.

When are 2018 training classes being scheduled?

Answer:  The Bureau of Elections is very busy scheduling training sessions for 2018.  The Train the Trainer program will be conducted in January and February.  Election Cycle Preparation (also fondly called County-by-County) will run from March through May.  And lastly, QVF will be rolling out new software with training sessions being held from February through April.  Once those schedules are finalized, a new session of Election Officials' Accreditation will be scheduled as well.
