North Ottawa Community Health System

• The vaccine clinic is self-scheduling by selecting a day and time.
• Please bring an ID and insurance card to your appointment.
• All minors must be accompanied by an adult to sign for consent.
• Second-dose appointments are on a walk-in basis. You will receive more information at your first-dose appointment. Walk-in times when you're second dose is due are 9 a.m.-3:45 p.m.

• Fill out the assessment to ensure you qualify.

Fully vaccinated people who meet the criteria are no longer required to quarantine following exposure to someone with COVID-19, providing the individual does not have any COVID-19 symptoms. The immune period begins two weeks after the completion dose. Michigan requires a 14-day quarantine.
