Ottawa County Vaccine Notification

The Ottawa County Department of Public Health (OCDPH) and participating health partners have limited appointments available for anyone aged 16 years or older to receive their COVID-19 vaccine at ONE of the locations and dates below. ONLY the Pfizer vaccine will be available to individuals 16-17 years of age--parental consent is needed and date of birth proof is required (driver’s license, state ID or birth certificate). If an appointment is scheduled for someone younger than 16 years, the appointment will be canceled. 

Please click on one of the red buttons below to schedule an appointment. This invitation does not guarantee an appointment. If you were not able to find an available time because all appointments were filled, we will notify you again via email/text message when more appointments become available. You may also click on one of the green registration buttons to sign-up for future notifications from partnering agencies when they have available appointments.

Fully vaccinated people who meet the criteria are no longer required to quarantine following exposure to someone with COVID-19, providing the individual does not have any COVID-19 symptoms. The immune period begins two weeks after the completion dose. Michigan is now requiring a 14-day quarantine. Individuals should keep their COVID-19 Vaccination Record Card to confirm their exemption from quarantine.  LEARN MORE

  • This is for the Pfizer two-dose vaccine. Please only schedule this first-dose appointment if you're able to receive your second dose on May 13 (to be scheduled after receiving your first dose). 
  • After you've successfully scheduled your appointment, you will see a green check mark confirming your appointment. You will also receive an email confirmation. If you do not get an email confirmation within a few minutes, please try scheduling your appointment again.
  • If all appointments have been filled, please check the other partnering agencies below for more availability.
