Ottawa County Vaccine Notification
The Ottawa County Department of Public Health (OCDPH) and participating health partners have limited appointments available for eligible people to receive their COVID-19 vaccine at ONE of the locations and dates below. Anyone who does not fit within the vaccine eligibility criteria at this time will have their appointments canceled.
If you already scheduled an appointment, please disregard this email. Otherwise, click on one of the red buttons below to schedule an appointment. This invitation does not guarantee an appointment. If you were not able to find an available time because all appointments were filled, we will notify you again via email/text message when more appointments become available. You may also click on one of the green registration buttons to sign-up for future notifications from partnering agencies when they have available appointments.

Tuesday, March 23
OCDPH Clinic at GVSU Holland Campus - 515 S Waverly Rd, Holland, MI 49423
- Please do NOT schedule a first-dose appointment at this time if you are not able to receive your second dose for April 20 (to be scheduled after receiving your first dose).
- After you've successfully scheduled your appointment, you will see a green check mark confirming your appointment. You will also receive an email confirmation. If you do not get an email confirmation within a few minutes, please try scheduling your appointment again.

SpartanNash Family Fare – Cherry Street Clinic, Hudsonville
- Scheduling link opens on Sunday for appointments.
- Register as a new user. Every individual MUST be registered for their own account, in their own name.
- Fill out the assessment to ensure you qualify.
- Search for an appointment using your Zip code.
- Select an appointment, check the box to accept and click confirm to lock in your appointment.
- You will receive an email confirmation confirming your appointment was scheduled with details about your appointment.
- If you have any questions or registration issues, please call one of the Family Fare or D&W store locations.

If you've signed up to receive vaccine notifications from multiple agencies, please be sure to only schedule your appointment at ONE location. Signing up for a vaccination appointment means a dose of the vaccine, a time-slot in the schedule and the staffing to deliver that vaccine are all reserved in your name. Signing up for vaccine appointments at multiple sites in West Michigan delays the vaccination process and hinders other people's access to the vaccine. Double scheduling can even risk wasting limited supplies because the vaccine must be used within hours of coming out of the freezer.
Avoid scams: Do NOT provide your social security number or other confidential information in the health department scheduling form or to callers stating they are from the health department. The OCDPH will NOT ask you for this information.
