Thank you for your patience. We have made a few adjustments to the MITW Annual Reporting information and process based on your feedback. A PDF template of the MITW Annual Reporting form is now available. You may use the PDF as a resource to preview the questions asked, but actual responses must be submitted using this Microsoft Form.

The Microsoft Form now also includes the PDF template as a resource.
