MGCB updates millionaire party application, dealer forms

Updated millionaire party forms now are available on the Michigan Gaming Control Board website.
The event application and event change forms were revised and a new form, Attachment B – Dealer List for Organizations, was added. Additionally, the dealer information form was revised for use by both organizations and suppliers.
Please use the updated forms for your next event.
