Part 213 Closure Reports and Certificates of Insurance
This email is to provide a reminder regarding Closure Reports submitted to the Department of Environmental Quality (DEQ) pursuant to Part 213, Leaking Underground Storage Tanks, of the Natural Resources and Protection Act, 1994 PA 451, as amended. The DEQ’s Remediation and Redevelopment Division (RRD) is finding that Closure Report submittals are frequently lacking the Certificate of Insurance required under Part 213. The Qualified Consultant's Affidavit section of the Closure Report Cover Sheet (form EQP4452) specifically states “Attached is a Certificate of Insurance demonstrating that I have obtained all the insurance required under Section 21312a(1)(c) and 21325 of Part 213.” Please assure that the Certificate of Insurance is included when submitting Closure Reports. This will avoid time lost in making contact to obtain the missing certificate, and will assure faster processing of the documents. Also, please be aware that the RRD will not consider a Closure Report submittal complete if the Certificate of Insurance is not included and may return the report to the submitter with notification that the submittal is incomplete.
