2023 Hazardous Waste User Charge
Invoice Announcement
Hazardous Waste User Charge invoice packages will be mailed during March 2023. Generators can expect to begin receiving the packets the week of March 13, 2023. The package will include an invoice and instructions. The pre-populated Site Identification (ID) Form (EQP 5150) will not be included; however, a link to the current Site ID Form will be provided in the instructions. Generators can access the form online, make any edits necessary, then print, certify, and submit the updated form with the invoice. Completed Site ID forms may also be emailed to EGLE-MMD-Site-ID-Reporting@Michigan.gov.
Reminders:
- The user charge invoice reflects 2022 hazardous waste generator status activity provided by the facility.
- Please do not submit hazardous waste manifests, as manifests are no longer included on the annual user charge invoice.
- Please do not submit liquid industrial by-products manifests or shipping documents.
- This is not a Biennial Reporting year for treatment, storage, and disposal facilities, or large quantity generators.
Invoice payments are due on or before April 30, 2023. Payment may be made by check, money order, or online with a Visa, Mastercard, American Express, or Discover credit card.
When making payment via credit card, please return the invoice or a copy of the invoice with a copy of the credit card receipt to EGLE-HWUserCharges@Michigan.gov or EGLE Materials Management Division, PO Box 30241, Lansing, Michigan 48909 (for overnight delivery, please use 525 West Allegan, Lansing, Michigan 48913).
If paying by check or money order, please return the invoice or a copy of the invoice with check payment to EGLE Cashiers Office - HWUC, PO Box 30657, Lansing, Michigan 48909.
