2022 hazardous waste user charge invoices
Hazardous waste user charge invoice packages will be mailed during March 2022. Generators can expect to begin receiving the packages the week of March 14, 2022. The package will include an invoice and instructions. The pre-populated Site Identification Form (EQP 5150) will not be included; however, a link to the current form will be provided in the instructions. Generators can access the form online, make any edits necessary, then print, certify, and submit the completed form with the invoice. The completed Site Identification Form can also be emailed to EGLE-MMD-Site-ID-Reporting@Michigan.gov.
Submittal of an updated Site Identification Form is only required if there was a change since the last submittal or if an updated Site ID Form was not submitted in 2021.
Reminders:
- The user charge invoice reflects 2021 hazardous waste generator status activity provided by the facility.
- Please return the invoice or an invoice copy with a check payment or with a credit card receipt when paying with a credit card.
- Please do not submit hazardous waste manifests, as manifests are no longer included on the annual user charge invoice.
- Please do not submit liquid industrial by-products manifests or shipping documents.
- This is a biennial reporting year for hazardous waste treatment, storage, and disposal facilities and large quantity generators.
Invoice payments are due on or before April 30, 2022. Payment may be made by check, money order, or online with a Visa, Mastercard, American Express, or Discover credit card.
