New interactive Site ID Form available for notifying of hazardous waste and liquid industrial by-products activities
The Michigan Department of Environment, Great Lakes, and Energy (EGLE), Materials Management Division (MMD), is pleased to announce the release of a new, interactive Site Identification Form (EQP5150) for use in notifying EGLE of hazardous waste and liquid industrial by-products activities. Now waste handlers can enter their data directly into the form and select the "Submit" button to send the completed form via e-mail to EGLE-MMD-Site-ID-Reporting@Michigan.gov and the e-mail contact listed on the form.
New facilities generating small quantity generator (SQG) and large quantity generator (LQG) amounts of hazardous waste are required to submit an initial notification. SQGs have to update their notification every four years, while LQGs have to update their notification every two years. Facilities accumulating greater than 11,000 pounds of universal waste, managing hazardous secondary materials, transporting hazardous waste or liquid industrial by-products, or accepting hazardous waste or liquid industrial by-products from off-site must also complete the Site ID Form notifying EGLE of their waste-handling activities.
"The new, interactive form will make it much easier for handlers to submit their data and for EGLE to process the information, making it available for public viewing through the Waste Data System," said Rich Conforti, Manager of the Data Management and Tracking Unit in MMD. "It's always a win-win situation when we can simplify our process making completing a task easier on both industry and staff, particularly during these pandemic times when more people are working remotely."
