Update on Evergreen Release 3 Rollout

To: All Evergreen Users

Evergreen Update:

We are pleased to announce that the following issues were resolved on 5/17/24:

  • The Behavior Management Plan (BMP) Attachments no longer copy forward when submitting a revised plan for committee approval, allowing the user to upload new versions.
  • Active Person Record Assignments are now displaying in the Person Summary for the Reportable Event Provider users.
  • The Person Centered Plan (PCP) Custom List Page ellipsis (three vertical dots) is no longer disappearing after the user attempts to select an inactive row.
  • PCP Key Milestones are now copying forward when revising an Annual Plan.
  • The Community Case Manager (CCM)/Brain Injury (BI) Care Coordinator roles are able to complete Change Type PCPs with a Change Reason of “90 Day Review”, as the Medical Professional options are populating under the Medical Review Date section now. 
  • Reportable Events with a category of Restraint are properly calculating Restraint Time Duration now. 
  • The “If Other” text fields in the EIS PCP Annual Medical Appointments have been migrated to the corresponding Evergreen Appointments note fields.  
  • The erroneous DD Eligibility forms which were created on BI Person Records during the Release 3 deployment have been removed.  
  • The ER Disposition error, "Value cannot be null Parameter 'input'" has been resolved, so Crisis Caseworkers can now complete these forms.  
  • The Person Guardianship/Conservatorship Status is now being displayed in the green Person Banner ribbon.
  • APS Reports can now be reassigned to a Case Worker from the originating District.

Provider Notices:

Please note that the processing of Prior Authorizations from 5/17/24 and 5/20/24 were delayed by the recent system outages. All PAs were sent to MIHMS for processing on 5/21/24.

Recurring Weekly Provider Meetings are being held every Friday from 8:00am to 9:00am. The meeting is focused on Service Implementation Plan, Reportable Events, and Prior Authorization Provider questions/issues, but is open to anyone who would like to listen. Common Questions/Issues:

Q: How do I submit a Reportable Event if Evergreen is offline for maintenance or down unexpectedly?  

A: We have created an offline (MS Word) version of the Reportable Event form which can be used to submit Reportable Events via email if Evergreen is unavailable. The form is attached to this email and can also be found in the Evergreen Help Center.

Q: How do I update my Service Implementation Plan (SIP) during the Annual Person Centered Planning (PCP) process?

A:  The assigned Case Manager/Care Coordinator will invite each Service Provider to complete a new (blank) SIP based on the waiver services linked to goals during the planning team meeting and then approved by OADS during the State Review.
