The Maine CDC opened orders for Nirsevimab on October 4, 2023 to Maine Immunization Program (MIP) providers and distributed approximately 450 doses of the 50mg presentation and 350 doses of the 100mg presentation to over 60 primary care offices across the state.

On Friday, October 13, 2023, U.S. CDC temporarily paused public health ordering of Nirsevimab due to high demand and limited supply. Nirsevimab has now transitioned to an allocation model and MIP has suspended ordering for all routine providers, with the exception of the 21 birth hospitals and tribal health centers.  

Due to the amount of existing Nirsevimab in provider's inventory that has gone unused, we are asking that providers administer it within 2 weeks of this communication. Any remaining inventory beyond this time frame, will be redistributed to provider sites that may have a more immediate need for it (i.e., hospital systems). MIP will be available to assist in transferring Nirsevimab, as necessary.

Recommendations for 50mg doses of Nirsevimab remain unchanged at this time.

We continue to recommend prioritizing available 100mg doses to infants at the highest risk for severe RSV disease. This includes all infants aged <6 months and select groups of older babies who have certain risk factors <8 months that are defined in the previous Maine CDC Health Alert Network (HAN) notice on October 24, 2023:  Limited Availability of Nirsevimab in the United States - Advisory (PDF).

Per the HAN, follow AAP recommendations for palivizumab-eligible infants aged <8 months when the appropriate dose of Nirsevimab is not available. U.S. CDC further recommends that providers suspend using Nirsevimab in palivizumab-eligible children aged 8–19 months for the 2023–2024 RSV season. These children should receive palivizumab per American Academy of Pediatrics (AAP) recommendations.
