Subject: ImmPact User Agreements

The Maine Immunization Program (MIP) will be requiring all Immpact Users to provide a signed ImmPact User Agreement by July 1, 2023, to remain active. We will honor User Agreements that are dated June 1, 2023, onward if it is the correct form, for the fiscal year of 2024 (July 1, 2023, through June 30, 2024). We will not be accepting any incorrect or outdated forms.
Please complete the ImmPact User Agreement with both the user’s signature and manager or designee’s signature and email to: ImmPact.Support@maine.gov.
