Health Home Application Now Open
The Health Home program is a key component of Maine’s Value-Based Purchasing strategy, a multi-pronged initiative designed to improve the healthcare system, improve population health, and reduce cost. Health Home practices partner with Community Care Teams and Behavioral Health Home organizations to serve MaineCare members with chronic conditions.
Participating practices receive a per member, per month payment to provide Health Home services to adults and children who are enrolled in the Health Home program. .
- The application is now open and will close Friday, December 11, 2020. Practices with more than one location must complete a separate application for each location. To apply, please access the online application.
- Practices that apply and qualify will be able to participate in Health Homes as of January 21, 2021. To review current MaineCare policy for Health Home Services, please see the MaineCare Benefits Manual, Chapter II, Section 91, Health Home Services.
- A webinar for interested primary care providers will be held on Monday, December 7, 2020 at 12:00 PM. To register for the webinar, please send your organization’s name, participant name, phone, and email address to the Value-Based Purchasing Unit.

Primary Care 2.0 Proposal Survey
Please provide your feedback on MaineCare’s Primary Care 2.0 proposal by completing a short survey by December 4, 2020. If you were unable to attend the Listening Session on November 18, 2020, please review the presentation on this proposal and share your feedback through the survey.
