Thank you for registering with the Baltimore City Health Department's COVID-19 Older Adult COVID-19 Vaccination Interest Form.  If you are receiving this message, we have recorded your interest in receiving the COVID-19 vaccine, either through our online interest form or by calling our MAP Hotline.

This email is to let you know that you will be receiving a call from one of our hospital partners in the next 2 weeks to schedule your vaccine appointment. Our hospital partner will make two attempts to reach you and will leave a voicemail message. When they call, please pick up, or return their phone call, so that you can receive your vaccine as soon as possible.
