Notice of Free COVID-19 Vaccination for Baker Residents 70 or Older
The City of Baker is pleased to announce our partnering with Southeast Community Health Systems to provide COVID-19 vaccinations for Seniors over 70!
Registration begins Tuesday, January 12, 2021; you can pick up registration forms at the City of Baker Municipal Building located at 3325 Groom Road, Baker, LA 70714.
Forms will be available either at the utility billing drive through or at the entrance.
Documents required will be a copy of your drivers license or state issued identification card and a copy (both front and back) of your health insurance card.
Forms along with copies of ID and insurance must be returned to the City of Baker no later than 12:00 noon on Thursday, January 14, 2021 in order to be pre-registered for a vaccination.
Vaccinations will be administered on Friday, January 15, 2021 from 1-4 pm at the parking lot of Miracle Place Church, 2080 Main Street Baker, LA 70714
On the day of the event, bring your ID and insurance card. Participants will remain in their vehicle throughout the process, including vaccine administration.
City of Baker personnel will verify that each participant has a Baker address by reviewing your ID.
Participants will remain in their vehicles, where they will then be given the vaccine.
The participant must wait 15 minutes then will be given the proof of vaccination card with the return date on it.
*It is necessary to get the 2nd dose of this vaccination in order to achieve maximum protection from COIVID-19..
Pre-registered persons will receive vaccines first.
If there are any doses left then they will be given out on a first come first serve basis.
