Great news! Thanks to your local health department and your Kentucky Employees' Health Plan, you now have local access to a free Vitality Check (health screening). If you complete an annual Vitality Check, HumanaVitality® will also reward you with up to 4,000 Vitality Bucks®! Schedule yours by calling the Marshall County Health Department at 270-527-1496. Where do I go to get my Vitality Check? Marshall County Health Department, 307 East 12th Street, Benton, KY. Members need to make an appointment. Find other locations near you by viewing the Vitality Check Locator. What is a Vitality Check? The Vitality Check measures your Body Mass Index (BMI), blood pressure, blood glucose and total cholesterol, and will provide a baseline as you make future wellness decisions. How do I get my 4,000 Vitality Bucks? Sign in to HumanaVitality at http://LivingWell.ky.gov and get started earning rewards! Your Vitality Bucks will be credited to your account within a few weeks. Can the health department come to my work location? Yes, if you have a group at your work location that would like to complete the screening, you may schedule an on-site visit with the health department. |
