Kids Sports Leagues = HumanaVitality bucks!

Your family can earn 100 Vitality PointsTM when your child joins a sports league or team. Each dependent child covered on your Kentucky Employees’ Health Plan can get rewarded through HumanaVitality for participation in two qualified sport leagues per year. Each sports league, with a minimum of eight games or matches, qualifies as a single activity. Find out more under the Vitality Kids section of the Get Healthy tab on your personal Humana Vitality page. Submit the sports league form online, or download the PDF to earn Vitality Points for lots of different activities. Just submit your form, along with a copy of the sports schedule or certificate of league participation, within 90 days of the league's completion date. Visit your local Department of Parks and Recreation or YMCA for sports leagues in your area. Don’t forget school sports leagues and teams – they count too! Register or log in to LivingWell.ky.gov and start earning points today!
