Take the Platinum Challenge!

As you conquer each level, you are entered to win bigger rewards!

The Platinum Challenge increases your chances to earn rewards as you advance through HumanaVitality℠!  This special, introductory challenge will be your guide through the status levels of HumanaVitality: blue, bronze, silver, gold and platinum. When you reach the goals of the Platinum Challenge, you will be automatically entered to win the status level prize package.

How do I become eligible for the prize package drawings?
Blue Status: Complete the health assessment before June 1, and you are eligible to win movie theater gift cards or a Nike gym bag.

Bronze Status: Complete the Vitality Check before July 1, and you are eligible to win an iPod nano, Nike + iPod sensor, $15 iTunes gift card and Nike gym bag.

Silver Status: Achieve Silver Status before Aug. 1, and you are eligible to win an iPod nano, Nike + iPod sensor, $15 iTunes gift card and golf bag.

Gold Status: Achieve Gold Status before Sept. 1, and you are eligible to win an iPod touch, Nike + iPod sport kit and Diamondback bike.

Platinum Status: Achieve Platinum Status before Sept. 1, and you are eligible to win an Xbox 360 and Kinect, Polar heart rate monitor and golf bag.

How do I get started?
If you are a Kentucky Employees’ Health Plan (KEHP) member and you have already logged in to HumanaVitality and taken your health assessment, you are already entered to win the June 1 health assessment prize.

If you have not already logged in to HumanaVitality, you have until June 1!  Go to http://LivingWell.ky.gov, click on the “Log-in” button under HumanaVitality to register and take your health assessment.  After you complete the health assessment, you are automatically entered to win.

What’s next?
The next drawing will be held on July 1 for HumanaVitality members who have received a Vitality Check.  Learn how to get your Vitality Check by visiting http://personnel.ky.gov/dei/wellness/VitalityCheck.htm.

The Platinum Challenge is administered by Humana Vitality, and Kentucky Employees’ Health Plan members are eligible to participate. Drawings will be held monthly in June, July, August and September.  Your information is completely confidential. No information is shared regarding your personal health.
