Regular screenings make it easier for you to manage and improve your health. HumanaVitality℠, your health plan’s new wellness program, recognizes the importance of regular screenings and wants to reward you up to 4,000 Vitality Bucks for completing an annual Vitality Check.

 

A Vitality Check includes a blood screening and biometric assessment, which measures your Body Mass Index (BMI), blood pressure, blood glucose and total cholesterol. You will earn 2,000 Vitality Points per year just for receiving your Vitality Check. If any of your values are in a healthy range, you can earn up to 2,000 additional Vitality Points.  

 

 

 

How do I get a Vitality Check?

Free Vitality Checks are available at the four Frankfort First On-site Clinics. Call today to schedule your Vitality Check, any time between 8 a.m. and 11 a.m.

Note: In order to get accurate results, the Vitality Check is a fasting screening. So, please do not consume any foods or beverages nine (9) hours prior to your morning appointment. Make sure to bring your state ID card and your health insurance card with you to the screening.

Is the Vitality Check free?

The Vitality Check is free to you as a Kentucky Employees’ Health Plan (KEHP) member at Walgreens Take Care Clinics, Kroger Little Clinics, Concentra® Medical Centers, participating Health Departments and First On-Site Clinics.  A co-pay may apply if you schedule a Vitality Check with your personal physician.  We recommend you ask your physician’s office if a co-pay will apply before scheduling your appointment.   

 

Why are health screenings important?

According to the U.S. Centers for Disease Control and Prevention, some of the leading causes of death in the United States can be detected early and cared for by having a regular health screening.  A Vitality Check is one of many regular screenings you may complete to help manage your health.
