LMS users attempting to upload documents this morning received error messages and, in some cases, may have seen a “ghost” document in the claim in which the upload was attempted. “Ghost” documents do not actually exist. All “ghost” documents have been removed from the database. Users who attempted to upload documents this morning are advised to check the claim file to be sure all documents uploaded appear in the record correctly and if not to upload the document again.
