As we enter the new year, we would like to remind you to please use the correct forms when filling out an application, waivers, fingerprints or any other forms relating to child care. It is important that we have the current forms used so that work may flow as efficiently as possible.
We would also like to remind you that if you fill out the forms electronically, you do not need to fill out the paper forms to send in. If you prefer to use the paper forms, please do not do an electronic form. Having both versions submitted slows the process down.
Please use the following link to access the current forms: https://www.kdhe.ks.gov/286/Forms-Applications. If you have older forms, please discard them, and use the current forms.
