Manage Health Conditions
In the U.S., 6 in 10 adults have a chronic disease like cancer, diabetes or heart disease. These chronic diseases and others are conditions that last one year or more and require ongoing medical attention or limit activities of daily living or both. Many chronic diseases can be prevented by eating well, being physically active, avoiding tobacco and getting regular health screenings.
Obesity is another chronic health condition that affects 1 in 3 adults and 1 in 5 children in the U.S. It raises the risk for several chronic diseases, including heart disease, type 2 diabetes and cancer. Other concerns include high blood pressure, high cholesterol, asthma, sleep apnea and joint problems.
Schedule an annual physical with your provider to review your body mass index (BMI) and screen for obesity. For reference, a BMI of 30.0 or higher falls within the obesity range and a BMI of 25.0 to up to 30.0 falls within the at-risk range.

Resources
The Invest In Your Health program offers a variety of tools and resources to help you manage your health conditions. These resources can help with prediabetes, diabetes, obesity, high blood pressure and more.

Schedule an annual physical with your primary care provider (PCP). The health plan pays for one physical every calendar year. Your PCP is a valuable resource who can guide you toward making informed decisions about your health, help you manage your conditions and assist you in establishing a preventive care plan tailored to your specific needs.

ActiveHealth provides unlimited coaching sessions by phone, virtually or in-person at some work sites. ActiveHealth coaches are certified professionals that are eager to help you reach and maintain a healthy weight.
- Anthem provides unlimited coaching through the Total Health, Total You care team. This team is especially great at helping medically complex individuals, and individuals working to get to healthy weight while managing chronic conditions.
- There are lots of free apps and tools such as myfitnesspal, fitbit and Lifesum to help you track your nutrition, weight and activity. Tracking is a great way to help you understand how your lifestyle could be impacting your weight.

Diabetes Self-Management Education (DSME) programs provide people with diabetes knowledge, skills and abilities needed to better manage their condition and improve their quality of life.
Work with a Health Coach to meet your goals...
The State of Indiana offers free Health Coaching through ActiveHealth to every employee and their spouse covered under the state health insurance plan.
A health coach can help you set personalized health goals and support you as you strive to reach them. Your health coach will provide expert help to make healthy changes in exercise, diet, sleep, mindfulness, work-life balance, tobacco cessation, weight management or other areas you want to focus on.
Eligible employees and spouses can earn $20 for completing a health coaching session (up to $100 total), through the wellness rewards program.
Call (855) 202-4219 to schedule an appointment. Health coaches are available from 9 a.m. to 9 p.m. ET Monday through Friday and from 9 a.m. to 2 p.m. on Saturdays by appointment.

Learning assignment for health plan members
Ever wonder how your medical plan costs are determined? Or what the differences between a deductible and coinsurance are? Or if there’s a cap on the medical expenses you pay annually? Get answers to these questions and more through a new learning module on medical insurance terms, costs and concepts with real-life examples.
All members of a State Personnel Department-sponsored medical plan will be enrolled in a training through SuccessFactors and will be required to complete by July 31, 2023. If this required training applies to you, you will receive an email with more details.
