Military Service Discharge Records (DD214's)

DD214 Request Form

Our office is the official keeper of military service discharge records that have been filed in Lake County by veterans since 1919.

A Request for Military Service Discharge Record form must be completed to receive a copy of any military service discharge record that has been recorded in Lake County, IL. Only qualified people listed on this form, with proper identification, are eligible to request a military service discharge record. Please visit our website at http://www.lakecountyil.gov/recorder to review or complete the form. Either fax or mail the completed form to the Lake County Recorder’s Office to receive FREE copies of your military paperwork.

If a Military Service Discharge Record was not previously recorded in Lake County, then you must request copies from the National Personnel Records Center (NPRC) in St Louis, MO by completing the SF180 form.

State ID Option for Military Veterans

veteran ID

Public Act 97-0739 allows veterans who have received an Honorable or General under Honorable Conditions discharge to pursue the veteran’s designation on their driver’s license or ID card. The veteran’s designation will help ensure military veterans living in Illinois receive the services and benefits for which they are entitled, including healthcare, education and employment assistance.

To obtain a veteran designation on your Driver's License or State ID card you must:

  • Be an honorably discharged U.S. Veteran.
  • Take your DD-214 or Certification of Military Service (NA Form 13038) to your local Illinois Department of Veterans Affairs' Office to have a certification stamp placed on your DD-214 or Form 13038.
  • Bring the certified DD-214 or Form 13038 to any Secretary of State Facility to obtain the "Veteran" designation on your Driver's License or State ID card.

If the veteran designation is added during the renewal of your Driver's License or State ID card, there is no additional cost. If a veteran chooses to add the designation before their scheduled renewal date, they may obtain an updated Driver's License for $5.00 and an updated State ID card for $10.00. Veterans over the age of 65 can obtain an updated State ID card at no cost.

To request a certified copy of your DD214, contact the Lake County Recorder’s Office at (847) 377-2575 or contact the Lake County Veteran’s Assistance Commission at (847) 377-3344 or veterans@lakecountyil.gov.
