The City of Evanston is operating a closed point of dispensing (POD) on Friday, April 9 where the Moderna vaccine will be administered.
Vaccine Eligibility
This POD is only for the following groups who live or work in Evanston:
- 1A
- 1B
- 1B+
- Food Service Workers
- Construction Trade Workers
- Religious Leaders
- Government Workers
- Those receiving a second dose that had a first dose on 3/12/21
Individuals who do not meet the above criteria will not be vaccinated at this event.
Do Not Share this Email
Since vaccine supply is limited, please do not share this email in order to ensure vaccine is distributed according to the phased approach.
Registration Required
Click here to select your appointment time. Registration will close at 6 p.m. on Thursday, April 8.
- All individuals seeking a vaccine must register. On-site registration will NOT be accepted.
- Green time slots mean there are appointments available. Red time slots mean appointments are not available.
- You will receive an email confirmation once you have successfully registered. If you did not receive an email confirmation, it means you did not complete the registration process and you will need to start over.
- Do not arrive earlier than your scheduled appointment.
- Do not arrive on site if you are sick or experiencing symptoms, such as fever or chill, cough, or shortness of breath or difficulty breathing.

I already got my first dose, why am I receiving this email?
Many individuals share an email address and they may be eligible for a vaccine at different times. We send the registration email to all eligible individuals in order to make sure we don't miss anyone. Please disregard this email if you already received your first dose.
The City of Evanston will be operating a closed point of dispensing for the COVID-19 vaccine each week. If you are unable to schedule an appointment for this event, you will be notified when the next event is available.
