Complete the COVID-19 Vaccination Contact Form
 Do you live or work in Evanston? Complete the City’s COVID-19 Vaccination Contact Form (español)! Completing the form will help you stay informed on vaccine distribution in Evanston, and will allow the City to contact you to schedule a vaccination appointment as soon as it is available to you.
In-home vaccinations: The City is working to reach adults 65 and older who are homebound due to medical conditions or are otherwise unable to attend the City’s COVID-19 vaccination events. If you fit this criteria or know someone who does, please complete the In-Home COVID-19 Vaccination Contact Form (español).
