Vaccination Contact Form Received

Greetings,

This message is to confirm that the City of Evanston’s Health and Human Services Department has received your Vaccination Contact Form. 

By completing the form, you have indicated that you are interested in receiving the COVID-19 vaccine, and will be contacted by the City’s Health and Human Services Department as soon as it is available to you. There is no further action needed from you at this time. 

Here are the next steps you can expect:

1. Await notification from the City. Please be patient. Based on the information you provided, the City’s Health and Human Services Department will notify you via this email address when the vaccine is available to you. (View sample notification.)
2. Register for your first vaccine dose. The City’s notification email will include a link to register for a 15-minute time slot to receive your first dose of the vaccine. Upon completing registration, you will receive a confirmation email and/or text message confirming the details of your appointment with a ticket to present for admission. (View sample ticket.)
3. Receive your first vaccine dose. Show up at your designated time slot at the location provided to receive your first vaccine dose. Please bring an ID if you are an Evanston resident. If you work in Evanston, please bring a work ID or documentation that shows you work in Evanston.
4. Await second notification from the City. The City’s Health and Human Services Department will send you a notification email to schedule an appointment to receive your second and final vaccine dose. You should expect to receive this email several days in advance of when the second vaccine dose is available to you. Pfizer doses are administered 21 days (or more) apart, and Moderna doses 28 days (or more) apart.
5. Register for your second vaccine dose. The email you receive will include a link to register for a 15-minute time slot to receive your second dose of the vaccine. Upon completing registration, you will receive a confirmation email and/or text message confirming the details of your appointment and a ticket to present for admission. 
6. Receive second vaccine dose. Show up at your designated time slot at the location provided to receive your second and final vaccine dose. Please bring an ID if you are an Evanston resident. If you work in Evanston, please bring a work ID or documentation that shows you work in Evanston.

What you can do now

Please continue to vigilantly follow public health guidelines while you await further notification from the City’s Health and Human Services Department. 

The City continues to follow a phased approach to vaccine distribution as outlined by the Illinois Department of Public Health and CDC. This approach is designed to ensure that while vaccine supplies are limited, those most fragile and vulnerable to severe illness and complications from COVID-19 are prioritized. Please note that the pace of vaccine distribution is entirely dependent on the number of doses the City receives on a weekly basis.
