The Evanston Health and Human Services Department is partnering with AMITA Health St. Francis Hospital to offer FIRST DOSE COVID-19 vaccinations to individuals aged 74 and older who work or live in Evanston.
Vaccine Eligibility
This is only for the following:

Individuals age 74 or older who live or work in Evanston OR
-
Individuals in Phase 1a who live or work in Evanston (health care personnel, paramedics, long-term care residents and staff)
Individuals who do not meet the above criteria will not be vaccinated at this event.

Since vaccine supply is limited, please do not share this email in order to ensure vaccine is distributed according to the phased approach.

- All individuals seeking a vaccine must register. On-site registration will NOT be accepted.
- Green time slots mean there are appointments available. Red time slots mean appointments are not available.
- You will receive an email confirmation once you have successfully registered. If you did not receive an email confirmation, it means you did not complete the registration process and you will need to start over.
- Do not arrive earlier than your scheduled appointment.
- Do not arrive on site if you are sick or experiencing symptoms, such as fever or chill, cough, or shortness of breath or difficulty breathing.
The City of Evanston will be operating a closed point of dispensing for the COVID-19 vaccine each week. If you are unable to schedule an appointment for this event, you will be notified when the next event is available.

I already got my first dose, why am I receiving this email?
Many individuals share an email address and they may be eligible for a vaccine at different times. We send the registration email to all eligible individuals in order to make sure we don't miss anyone. Please disregard this email if you already received your first dose.
When will I get my second dose? Can I get my second dose here?
This vaccination clinic is NOT for second doses. You will receive a separate registration link specific for your second dose.
