The City of Evanston is operating a closed point of dispensing (POD) on Tuesday, February 2 where 1st doses of the Moderna vaccine will be administered.
Vaccine Eligibility
This POD is only for the following:

Individuals age 79 or older who live or work in Evanston OR
-
Individuals in Phase 1a who live or work in Evanston (health care personnel, paramedics, long-term care residents and staff)
Do Not Share this Email - Individuals not meeting the above criteria will not be vaccinated at this event.
Since vaccine supply is limited, please do not share this email in order to ensure vaccine is distributed according to the phased approach.
All individuals seeking a vaccine must register. On-site registration will NOT be accepted.
Please do not arrive earlier than your scheduled appointment.
Do not arrive on site if you are sick or experiencing symptoms, such as fever or chill, cough, or shortness of breath or difficulty breathing.
The City of Evanston will be operating a closed point of dispensing for the COVID-19 vaccine each week. If you are unable to schedule an appointment for this event, you will be notified when the next event is available.

You will receive an email confirmation once you have successfully registered. If you did not receive an email confirmation, it means you did not complete the registration process and you will need to start over.
