Complete the City's Vaccination Interest Form
Help the City better understand local vaccine demand and support ongoing planning and distribution efforts by completing Evanston’s Vaccination Interest Form. (View Spanish form.)
All individuals 16 years and older who live OR work in Evanston are encouraged to complete the form to receive regular updates on the vaccine process and when and where the vaccine will be available to them. This is not a vaccine registration form.
Other vaccine updates
- Through January 6, the City has received 3,550 COVID-19 vaccine doses and provided 2,765 doses to Evanston hospitals, healthcare providers and paramedics.
- Starting today and continuing into next week, the City anticipates administering an additional 500 vaccine doses to non-hospital and community-based healthcare workers at a closed vaccination event.
- As more vaccines are received, the City will continue to host closed vaccination events throughout the winter to distribute the vaccine to priority groups, including frontline essential workers, those 65 years and older, and those at higher risk for severe illness from COVID-19.
- Vaccines are being administered to residents and staff of long-term care facilities as part of a federal partnership with CVS and Walgreens.
- Although the vaccine supply is currently limited, it is anticipated that the supply will continue to increase in the coming weeks and months.
