Communities of Care Co-Location Program

RFP #COMPADM26005 — This procurement seeks to establish rural Co-Location Sites where multiple providers work together in one place so residents can access a broad range of services that address health and health related social needs in a single visit. This project will also transform Rural healthcare delivery by providing an opportunity for Co-location Sites to share costs and sustain care long term in their local communities. The Co-Location Sites will bring together at least three separate and distinct service delivery entities, each offering different types of initiatives or services, which will collaborate in integrating services within a single rural location. The purpose of these co location sites is to expand and sustain access to care in rural communities, foster new partnerships among local providers, improve chronic disease outcomes, and embed preventive and chronic disease management services so individuals can access multiple coordinated appointments in one place.

Mobile Integrated Health 

RFP #PHTHOET27001 — This procurement seeks to select the most qualified applicants to initiate or expand Mobile Integrated Health (MIH) services within defined rural areas across Iowa. The purpose of developing Mobile Integrated Health projects is to initiate or expand treat-inplace care, telehealth-enabled clinical assessments, chronic disease management, post-discharge follow-up, prenatal and postpartum home-based support, medication reconciliation, and other community-based clinical interventions for Rural Iowans.  

Maternal Health Hub & Spoke Program

RFP #COMPADM26006 — This procurement seeks to select the most qualified applicants to serve as maternal health Hub and Spoke sites. The purpose of developing Hub and Spoke sites is to sustain current maternal health care in Iowa and increase access to and availability of high-quality prenatal care, labor and delivery services, postpartum care, mental health support, and behavioral health support for those pregnant and postpartum in Rural Iowa. Through the Healthy Hometowns Rural Health Transformation Program, Iowa is working toward innovative and transformative models of healthcare delivery to best serve rural Iowans.  

High-Risk OB & Neonatal Transport Initiative

RFP #PHTHOET27002 — This procurement seeks to select the most qualified applicants to implement the High-Risk OB and Neonatal Transport Initiative. This initiative will strengthen Iowa’s emergency maternal and neonatal care system by funding equipment purchases, non-reimbursable clinical services, workforce initiatives, continuing education and training.
