IRIS users,
Healthcare providers may receive requests from individuals for their immunization records. IRIS enrolled users can print and provide patient immunization records from IRIS. Instructions to generate a patient immunization history report from IRIS are available here. The patient immunization history report includes the patient’s entire immunization history and can be given to patients requesting immunization records. All IRIS users must comply with Chapter 7 of the Iowa Administrative Code, the IRIS Security and Confidentiality Policy, and the IRIS user agreement when releasing immunization records.
