The Iowa Board of Pharmacy (“Board”) took the following actions at its most recent meeting held on September 18-19, 2018:

Sheila Schleeper, Pharmacist.  The Board issued a Notice of Hearing and Statement of Charges, charging Pharmacist Schleeper for having disciplinary action taken against her Illinois pharmacist license.  A hearing is scheduled for November 13, 2018.

Alan Bollinger, CSA Registrant.  Dr. Bollinger entered into a Settlement Agreement and Final Order with the Board to resolve a disciplinary charge for having disciplinary action taken against his medical license.  The Order restricts his CSA registration in a manner that mirrors the prohibition on prescribing, administering, or dispensing controlled substances for the treatment of chronic pain imposed by the Iowa Board of Medicine.

David Sinnwell, Pharmacist.  Pharmacist Sinnwell entered into a Settlement Agreement and Final Order with the Board to resolve a disciplinary charge for failure to complete the required continuing education.  The Board issued a citation and warning, imposed a $750 civil penalty, and required Pharmacist Sinnwell to complete 60 hours of continuing education. 

Stacy Cook, Technician.  Technician Cook entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the Board.  Technician Cook voluntarily surrendered her technician registration to resolve a disciplinary charge for diverting prescription drugs.

Ankeny Pharmacy, Pharmacy.  The Board denied Ankeny Pharmacy’s request for early release from probation.  Pursuant to a Board Order dated May 4, 2016, Ankeny Pharmacy is on probation for a period of four years.  The Board voted to reduce the required frequency of controlled substances audits from monthly to quarterly and to reduce the required frequency of self-inspections from quarterly to every six months.

Mariam Amir Fahmy Wanis, Intern.  The Board granted Intern Wanis’ request to reduce the number of required internship hours from 1500 to 1200 in recognition of her pharmacy experience.

NuCara Pharmacy, Telepharmacy Site Application.  The Board granted the applicant’s request to waive the 10-mile restriction to allow the proposed telepharmacy site to open in Brooklyn.  The Board’s proposed decision is subject to mandatory review by the director of public health.

Timothy Schirm, Pharmacist Applicant.  The Board approved Pharmacist Schirm’s application for licensure by license transfer/reciprocity.

Brandon Wayne Black, Nonresident PIC Applicant.  The Board approved Pharmacist Black’s application for a nonresident pharmacy pharmacist in charge registration.

Rules.  The Board voted to adopt ARC 3848C.  The Board voted to file a Notice of Intended Action to delete 657—4.9(3).  The Board voted to file a Notice of Intended Action to amend 657—8.35 and 657—13.16.  The Board voted to file a Notice of Intended Action to amend 657—39. 

 

 

 
Our Board is comprised of five pharmacist members and two public members. The Board is responsible for regulating the practice of pharmacy and the legal distribution and dispensing of prescription drugs and precursor substances throughout Iowa. The responsibilities include, but are not limited to, licensing, renewal, and discipline of pharmacists, pharmacies, drug wholesalers, pharmacist-interns, pharmacy technicians, and persons or businesses involved in the distribution of controlled substances in Iowa, and continuing education for pharmacists.
