This notice is to inform you that you are required to submit the Sworn Statement in Lieu of Annual Statements for Issuers of Donor Annuity Agreements attesting that the issuer has met all requirements of the law for the following companies:

Overview
In accordance with Rule 69O-202.012, Florida Administrative Code, companies are required to submit Sworn Statement in Lieu of Annual Statements for Issuers of Donor Annuity Agreements to the Florida Office of Insurance Regulation (OIR) on an annual basis.
Filing Deadline
All companies authorized to write donor annuity agreements in Florida must submit the form.
Filing Deadline: 60 days after the end of the company's fiscal year
Scope Period: January 1, 2022 – December 31, 2022
Penalty: Failure to comply may result in administrative action.
How to File
Each company must submit its filing on an individual company basis.
Please use the Insurance Regulation Filing System (IRFS) for filing submission.
