This notice is to inform you that you are required to file Life & Health Gross Annual Premium and Enrollment (GAP) data for the following companies:

Data Call Overview
In accordance with section 627.9175, Florida Statutes, companies are required to report GAP data to the Florida Office of Insurance Regulation (OIR) on an annual basis.
Filing Deadline
Each health insurer, prepaid limited health services organization, and health maintenance organization shall submit, no later than April 1 of each year, to OIR information concerning health and accident insurance coverage and medical plans being marketed and currently in force in this state.
Filing Deadline: April 1, 2021, 11:59 p.m. EST
Reporting Dates: January 1, 2020 – December 31, 2020
Each company must submit its filling on an individual company basis.
Penalty: Insurers must submit data in accordance with sections 624.316, 624.3161, and 627.9175, Florida Statutes. Failure to comply may result in a referral to OIR’s Life and Health Market Regulation Unit for further action.
