The COVID-19 Vaccination Center located at the UF/IFAS Extension Clay County T. Jesse Godbold Building, 2493 State Road 16 West, Green Cove Springs, FL will be closing on Friday, May 7th.

Final 1^st doses will be offered on Friday, April 16, 2021, with all seconds doses continuing at the Clay County Vaccination center located on SR 16, until Friday, May 7^th.

Starting Monday, April 19^th individuals who are interested in scheduling their 1^st dose of COVID-19 vaccine can schedule an appointment through the DOH – Clay Bear Run clinic by calling (904) 529-2800.
